I need a center mount drawer slide. where would I be able to purchase it? I have already tried hardware store?

I purchased it from JC penneys about 8 years ago. I tried through them to get a replacement. I was told they would send a repairman to our home. Unacceptable. I am a do it yourselfer and would prefer to find it and put it in on my own. Any suggestions?
